There are 459,000 social-run medical institutions, accounting for 46% and 21,000 hospitals, accounting for 63.5%. Social-run medical institutions have become an important part of Chinas medical service system.
Under the circumstance that the total quantity of high-quality medical resources is relatively insufficient and unevenly distributed in our country, social medical service is an important force to increase the effective supply of medical resources and meet the diverse health service needs of the masses at various levels.
However, the overall level of social medical service is not high, and there are shortcomings in medical technology, subject level, service quality and brand reputation. In some places, the implementation of policies is inadequate, non-standard, and the supervision is not perfect. It is urgent to further improve policy measures and increase the encouragement, support and promotion of standardized development of social medical services.

Recently, 10 departments, including the National Health Commission, the National Development and Reform Commission, the National Medical Insurance Bureau and the China Banking Insurance Regulatory Commission, jointly issued the Opinions on Promoting the Sustainable and Healthy Development of Social Medical Services (hereinafter referred to as Opinions), which is a green light for social medical services.
The Opinion puts forward 22 policy measures from six aspects: increasing government support and simplifying examination and approval services. Including strictly controlling the number and scale of public hospitals, leaving enough space for social medical development, for-profit social medical institutions, including clinics and other small medical institutions, can enjoy preferential tax policies for small and micro enterprises according to regulations, support and encourage social medical institutions to participate in the evaluation of hospital grades, the government does not make planning restrictions on the total volume and spatial layout of social medical areas, and improve medical insurance support policies. And so on.
On June 12, at the State Council policy briefing, Wang Hesheng, deputy director of the State Health and Health Commission, said that with the aging of the population in China, the demand for rehabilitation, nursing and health care services will increase substantially, providing a broad space for social development. In addition, in some areas that are in short supply, such as pediatrics, psychiatry, obstetrics and gynecology, ophthalmology, medical cosmetology and so on, there is great potential for social medical services.
Medical examination, hemodialysis, medical imaging and so on are also areas in which social medical services can be focused. With the increasing demand of peoples health service, the development prospects of social medical service will be better and better. Wang Hesheng said.
Strictly Control the Number and Scale of Public Hospitals
Opinions began to highlight the strict control of the number and size of public hospitals, to leave enough room for social development of medical services.
This statement is not the first to mention, as early as 2014, the former National Health Planning Commission issued an emergency notice to control the rapid expansion of public hospitals. Among them, it said that in recent years, the scale of public hospitals in China has been expanding too fast, some hospitals are too large, there are problems of extensive development such as pursuing bed size, competing to purchase large-scale equipment, neglecting the internal management and mechanism construction of hospitals, which has increased the unreasonable growth of medical expenses, squeezed the development space of grass-roots medical and health institutions and non-public hospitals, and is not conducive to the improvement of hospitals. Service quality and management level.
The 13th Five-Year Plan on Health and Health issued in early 2017 mentioned that we should promote the scientific development of public hospitals and control the rapid expansion of public hospitals.
According to the Statistical Bulletin on the Development of Health Careers in China in 2018, there were 12032 public hospitals in 2018, 265 fewer than in 2017, and the proportion of beds in public hospitals also declined. However, the average outpatient and hospitalization expenses in public hospitals increased by 3.7% and 2.2% respectively compared with the previous year, and the number of beds in medical and health institutions per 1,000 population increased from 5.72 in 2017 to 6.03 in 2018. Statistical data released by the institutions show that the number of beds in county hospitals in China has reached 3000, and it is not uncommon for county hospitals with beds higher than 1000.
How to reconstruct the composition of beds instead of simply building the Central Hospital in big cities into 3,000 or more beds? This is not only the unreasonable distribution of social resources, but also the deformity of the social development space. Opinions pointed out that public hospitals in the future can not take the road of quantity and scale development, but to find their own basic role in security positioning. Zhang Chengyu, president of Shanghai Harmonious Family New Town Hospital, told economic reporters in the 21st century.
In terms of taxation, the social medical service has also been given preferential policies. Dr. Han Xiaohong, founder of Ciming Medical Examination and chairman of Dr. Han Medical Group, told reporters that private medical institutions should levy 25% tax according to the enterprise standard, but public hospitals are exempted from enterprise income tax. Therefore, the same medical examination service has a big price difference between public hospitals and private medical institutions.
The Opinion clearly states that for-profit social medical institutions, including clinics and other small medical institutions, can enjoy preferential tax policies for small and micro enterprises according to regulations. Social medical service providers may apply for recognition of high-tech enterprises in accordance with regulations and enjoy corresponding tax preferences.
Solving the Shortage of Medical Personnel in Society
The shortage of qualified personnel is a major problem faced by the society in running medical institutions. Excellent and senior doctors are mainly concentrated in large public hospitals or third-class hospitals.
Han Xiaohong said that money is not the most important thing in the development of social medical services. If the market is clearly positioned, demand is there and capital is good, the most important difficulty is the lack of talents, which should be released as soon as possible, so that socialized companies have the opportunity to participate, otherwise it can not be promoted.
The Opinions put forward that the electronic registration system for doctors and nurses should be implemented in an all-round way, the regional registration system for doctors should be implemented in an all-round way, and the management of regional registration for nurses should be promoted. Supporting and standardizing multi-agency practice of physicians.
It is understood that China has issued a management document on standardizing multi-point practice of doctors since 2014, scientifically allocating multi-point practice of doctors, and transforming the registration of medical practicing institutions into regional registration. In 2018, there were 160,000 doctors practicing in many institutions, and a considerable part of them entered the medical institutions run by the society.
The Opinion also seeks to break down the barriers that hinder the development of social medical services and achieve equal treatment for both social medical services and public medical institutions.
Among them, the professional and technical personnel of public medical institutions and public medical institutions participate in the evaluation of professional titles equally. The selection of medical research project undertakers adheres to the principle of openness and equality, and the nature of medical institutions can not be restricted. All kinds of medical institutions can voluntarily submit fixed-point applications for basic medical insurance. They can not take the sponsor and the nature of operation as the preconditions.
It is worth mentioning that the Opinion also mentions the exploration of multiple modes of cooperation in medical institutions. We will support the cooperation between public hospitals and public hospitals in medical services, subject construction and personnel training.
Zhang Chengyu said that in the past, this aspect was not clear. Public hospitals dared not cooperate with social hospitals for fear of violating the red line, but now encourage social hospitals to cooperate with public hospitals, minefields do not exist.
